This form is not required, if you have already completed a Membership form, which includes the waiver language as part of that form. If another family member filled out the membership form, you can complete another adult waiver below.

To volunteer at Simple Gifts Community Garden, each adult (18 and over) must complete and submit the waiver form below. One parent or guardian can complete the minor release section to cover all minors in the family.
